BMJ Case Rep Published online: [please include Day Month Year] doi:10.1136/bcr-2015210365 DESCRIPTION Coronary artery dominance refers to whether the posterior descending artery (PDA) and posterolateral vessel (PLV) originate from the right coronary artery (RCA; 80%), left circumflex artery (LCX; left dominant 8–10%), or both (codominant). A vessel is considered super-dominant when it is extremely large and supplies the territory that is normally supplied by the other vessel. We present an interesting case of a 56-year-old hypertensive woman with no other significant medical or surgical history. The patient presented with unstable angina and was taken up for diagnostic angiogram, which incidentally revealed the presence of a super-dominant LCX artery; the artery supplied the PDA and PLV branches rendering the

Clinical Implications of Congenital Absence of Circumflex Coronary Artery

Introduction: Coronary artery anomalies are rare clinical entities reported in 0.6% to 5.6% of diagnostic coronary angiographies. Anomalous origins of coronary arteries from distal segments are rarely reported. Presented herein is a coronary anomaly in which the circumflex artery (CX) originated as a continuation of the posterolateral branch of the right coronary artery (RCA) w...

[Localization and quantification of myocardium at risk by myocardial perfusion SPECT during coronary artery occlusion].

INTRODUCTION AND OBJECTIVES The aim of this study was to analyze the distribution and extent of the myocardium at risk using polar maps obtained with myocardial perfusion SPECT. Myocardial perfusion of territories irrigated by the left anterior descending (LAD), right coronary (RCA) and left circumflex artery (CX) was studied with the help of a technetium-radiolabeled tracer during occlusion of...

Congenital Anomaly of Single Dominant Right Coronary Artery with Hypoplastic Left Coronary Artery.

UNLABELLED With the popularization of new imaging technology, more people are deciding to undergo non-invasive studies such as multidetector computerized tomography (MDCT) before receiving coronary angiography. For this reason, coronary anomalies of coronary artery are being encountered more frequently. We here report a 68-year-old male presenting with typical angina. The MDCT images suggested ...

Reduction in myocardial perfusion territory and its effect on the physiological severity of a coronary stenosis.

A man aged 56 years presented with worsening exertional angina. Coronary angiography showed a moderate-tosevere stenosis in the midleft anterior descending artery (LAD) [Figure 1a]. The right coronary artery (RCA) was chronically occluded (Figure 1b). A coronary pressure wire assessment of the mid-LAD lesion yielded a fractional flow reserve (FFR) of 0.72, indicating this to be functionally sig...

Significant narrowing of the circumflex artery leads to worse outcomes than right coronary artery narrowing in patients with anterior myocardial infarction treated invasively

BACKGROUND Occlusion of the circumflex artery (Cx) often does not present signs in the ECG. It can lead to delayed angiography during ST-elevation myocardial infarction (STEMI). The aim of this analysis was to determine if Cx narrowing is related to diverse outcomes in comparison with right coronary artery (RCA) stenosis in patients with STEMI, treated with percutaneous coronary intervention (P...
